Wound Care Nurse - LPN/LVN

Role Summary

Our client is searching for a Wound Care Nurse which requires a combination of effective strategic thinking and relentless hands-on execution.  This person will provide direct patient care in the outpatient wound care setting in collaboration with the supervising registered nurse (RN) and wound care provider as permitted by state scope of practice and hospital guidelines.


Key Result Areas

  • Collaborates with physicians, RNs, HBO technicians, and other healthcare professionals to coordinate comprehensive patient care.
  • Contributes to the interdisciplinary plan of care in partnership with the RN and wound care provider.
  • Provides wound care in alignment with established clinical competencies, training standards, and evidence-based practices.
  • Delivers care grounded in scientific principles, nursing theory, and best practices.
  • Accurately implements provider orders within appropriate timeframes.
  • Monitors patient conditions, identifies changes, and responds appropriately, escalating concerns to the supervising nurse or provider as needed.
  • Prioritizes nursing interventions based on patient acuity, needs, and desired outcomes.
  • Demonstrates proficiency in both core and unit-specific clinical skills.
  • Identifies ongoing patient care needs and supports discharge planning efforts.
  • Performs hyperbaric oxygen therapy (HBO) responsibilities in accordance with policies and business needs.
  • Educates patients, families, and caregivers on care plans and post-treatment instructions.
  • Ensures timely and accurate documentation of all patient care activities in the appropriate EMR systems.
  • Maintains complete and compliant records in accordance with hospital and departmental policies.
  • Utilizes hospital and company systems to support patient scheduling and care coordination when needed.
  • Adheres to all HIPAA, privacy, and confidentiality standards.
  • Follows patient safety guidelines and infection control protocols at all times.
  • Supports efficient use of resources, including equipment and medical supplies.
  • Participates in quality improvement initiatives and infection prevention efforts.
  • Performs phlebotomy and bedside diagnostic procedures, as applicable and under appropriate supervision.
  • Processes and manages computerized patient care functions, including admissions, discharges, and test results.

 

  • Upholds company policies, procedures, and core values in all aspects of work.
  • Contributes to achieving organizational goals and overall mission.
  • Performs additional duties as assigned.


Education and Technical Skills:

  • Must have and maintain a valid BLS certification from the American Heart Association.
  • Associate degree in nursing, Bachelor of Science in Nursing required for Magnet Hospitals
  • Must have and maintain a valid Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the state in which the Nurse will work.
  • Prefer prior experience working with wound care patients.
  • Prior experience with billing and coding preferred.

Physical Requirements:

This position requires periods of time in which sitting, standing, use of hand and foot motion, vision, hearing, summarizing, focusing with frequent interruptions along with other physical, sensory, and cognitive sensory functions are required.

  • Must have full range of body motion.
  • Must be able to lift a minimum of 50 lbs.
  • Vision abilities required include close vision, distance vision and color vision.
  • Must be able to speak and hear clearly.
  • Must have the ability to stand, sit, walk, bend, lean, and squat for long periods of time without hindrance.
  • Must be able to maintain dexterity when pulling, pushing, and gripping emergency carts and stretchers.
  • Must be physically able to perform CPR.
  • Must perform all physical job duties accurately in a fast-paced environment with frequent interruption.
